Patients are seen by appointment only:
We will schedule an appointment at your convenience whenever possible.
If the preferred doctorís schedule is full, you may be given the
opportunity of seeing another physician or nurse practitioner.
We require 24 hour
notice for cancellation of an appointment.
You may be charged a $25.00 fee if
you do not give notice.
When your child is sick:
A receptionist will take the patientís name, your name and
phone number. A nurse will call you back after your childís chart is
pulled and given to the nurse. In order to be fair to all our patients,
the nurse returns calls in the order in which they were taken. The nurse
will help answer your questions and schedule an appointment if your
child needs to be seen.
What to do in an emergency:
Emergencies can happen at all hours. When they occur quickly gather
facts. Be ready with information about temperature, alertness, vomiting,
diarrhea, bleeding or other symptoms.
For a life-threatening emergency - DIAL 911
If you donít have a life-threatening emergency, a nurse is available to
manage your call. One of our pediatricians is available at all times.

Children needing services without direct physician contact:
such as strep screens, immunizations, weight checks and suture
removal, are worked in between scheduled appointments during the hours
of 9 am-11 am and 1 pm - 4 pm. Please call and schedule if these
services are needed.

If you have an urgent concern after office hours, use the phone number
listed in the gray box above. A nurse will assist you with your
situation and determine if your child needs to be seen promptly by a
Canceling an appointment:
If you are unable to keep your appointment, please cancel as
far in advance as possible. We appreciate the opportunity to utilize
that time for another patient.
We encourage every child to be adequately immunized. We offer
immunizations to all our patients. Please call for schedule.
Be sure to check out with the receptionist when you leave and schedule
any future appointments that you may need.
We request payment at the time of the visit. This reduces the total cost
of your health care. If
your visit is covered by a participating health insurance plan, we will
accept your co-pay and file your insurance claim. We will need a copy of
your insurance card. If you have a medical card, please have it with you
at your appointment. Questions about your account may be directed to
our business office at 235-0104.
1.5 % per month interest will be charged on all balances greater than
Prescription and Refills:
When you need a prescription refilled, please call during office hours
and talk to your doctorís nurse. She will ask your doctor if it can be
called into your pharmacy or if the doctor will need to examine your
child. Please allow at least 3 days for written prescription requests or
prescriptions for mail out programs.
Day Care and Camp Forms:
Your child must have a current complete physical on file with our
office. Please allow 3 working days for these types of forms to be
filled out. You may drop off the forms during regular business hours or
mail them in. You may pick them up 4 days later or you may enclose a
self-addressed stamped envelope and we will mail them back to you.
Copies of medical records:
If you need copies of your medical records to be sent to your insurance
company or other third party requesters, we ask that you fill out a
release form giving us permission to send this personal information.
A fee may be charged for copies sent to parties other than another
Information Update Sheets:
We must have a current signature on file for insurance and
information release purposes. Therefore, we will ask you to fill out a
new information update sheet each year. This is to ensure that we have
current address, phone number, insurance and other information so we can
reach you when necessary and promptly file insurance claims. We thank
you for your assistance in keeping our files current when information
Our doctors are currently on staff at both St. Francis and Stormont-Vail
hospitals. For newborns, the nursery staff will notify us after your
baby is born. You must let us know if your insurance requires you to use
a specific hospital. The hospital is completely separate from our office
and bills for its services only. Our fees for care provided by our
physicians will be mailed to you and/or your insurance company after
your child is discharged. Effective
December 28, 2008 our doctors have dropped their staff privileges at St.
